Based on the information obtained during today's visit, it was determined that Title 22 Regulation section 101229(A)(1) (Responsibility for providing Care and Supervision) was violated. The Regulation states that no child(ren) shall be left without the supervision of a..... 1 The following plans of corrections have been implemented and discussed during today's visit: 1. Implemented a policy requiring management to monitor all transitions. 2. Established triple headcount procedures for all transitions. teacher at any time...Supervision shall include visual observation. It was determined that the subject child was inside the classroom by themselves for a brief period of time, defined as a matter of seconds. The way the child got back into the classroom is only based off of assumptions. 8 3. Conducted staff trainings on appropriate transitioning practices. 4. Completed additional staff training on use of CSRs. Facility agrees to submit documentation of new proceedures and trainings to LPA no later than 02/02/2026.
Based on pertinent interviews conducted and documentation reviewed, it was revealed that the facility operated out of ratio in the infant classroom for a limited amount of time. It was revealed that this incident occured during drop-off time in the morning. 8 Facility also agrees to provide the Department with a written backup plan for situations where staffing is low. Facility agrees to submit the plan of correction to the Department by 12/02/2025, by 5:00 PM.
Facility has already implemented a new "body check" policy for all infant and toddler children. It was stated that this new policy requires teachers to conduct further body checks of children during diaper changes, to check for bruises. It was also stated that the facility moved furniture in the toddler class. During pertinent interviews, it was found that no staff was able to explain the cause of the bruising, which proves the reported allegation to be true. 8 Facility also agrees to conduct a supervision training for all infant and toddler teachers. The training shall include the understanding of constant supervision for children, including nap time. Facility agrees to submit the plan of correction to the Department by 12/05/2025, by 5:00 PM.
Interim Director agrees to submit a written statement that demonstrates how the facility plans on providing adequate care and supervision for all children and how to avoid them from climbing on the classroom sinks and tables. Director agrees to submit plan of correction to LPA via email by 09/04/2025. Pertinent interviews stated that teachers do observe these moments, but are not taking enough measures to prevent this behavior from continuing. Also, during today's visit, several children were observed to be climbing into subject the classroom's sink. 8 Interim Director also agrees to conduct a training with all staff members, regarding the responsibility to provide appropriate supervision. Director agrees to submit a copy of the training/meeting’s agenda and sign-in sheet once the training is completed.
Based on the staff member's own admission and information disclosed by the witness it was stated that the staff member pushed a child, but not with malicious intent. It was stated that the intention was to redirect the child from climbing on another child. 8 Interim Director also agrees to conduct a training with all staff members, regarding the responsibility to protect Personal Rights of all children. Director agrees to submit a copy of the training/meeting’s agenda and sign-in sheet once the training is completed.

Based on observation and interview, the facility had debris on the floor of the infant classroom including food, dust, discolored marks on the floor, and dark or discolored marks on a significant portion of the walls in the infant’s reach. Infants were observed crawling on the floor and putting their hands and feet in their mouth. Based on interviews obtained, it was determined that the child was able to walk out of the Infant-Toddler's main gate, as it was left open by the gardener. 8 Further, Director also agrees to submit a written statement to LPA stating the new proceedures that will be in place in regards to supervision and ensuring that the facility gates are to remain shut at all times. Director agrees to submit plan of correction to LPA via email by 08/19/2024.
Based on observation, the licensee did not comply with the section cited above in that poisons (bleach) was not locked, which poses an immediate health, safety or personal rights risk to persons in care. While touring the door to the laundry room in the toddler classroom was not locked and inside the laundry, on top of the washer was a bottle of bleach.

Based on interviews conducted and observations made by LPA, child was able to leave the classroom unattended and wander to the playground where the child was found by another teacher. This is an immediate risk to the health and safety of children in care.
Based on information received, the Child care center had an incident of an infant that was left unsupervised and failed to report this to CCL. This poses a potential health, safety or personal rights risk to persons in care
Personnel Requirements - Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not being met as evidence by the observation made by the LPA of three staff members that were providing care for infant/toddlers however, 1 Director was able to associate all three staff members, via Guardian, during the inspection. Director agrees to complete the Guardian training that is available on their website. Also submit a written plan to the Riverside Child Care Regional office by 6/29/2022. none of the three staff were associated to any of the facility's three license. This poses an immediate health, safety or personal rights risk to the children in care. A $1500.00 civil penalty is being issued as a result of this citation/deficiency 8
