Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. 1 The Director agreed to submitt the shot records as soon as possible no later than 3/12/25 by email. This requirement is not met as evidence by: Based on the observation and the record review there are 2 staff that do not have MMR, DTaP, and TB test, which poses a potential health, safety, or personal rights risk to persons in care. 8

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. 1 The Director agreed to submitt the Mandated Reporter as soon as possible no later than 3/12/25 by email. the incident occurring on 12/2/2022, child # 1 had inappropriate contact with child # 2. Staff #1 and Staff #2 supervising child # 1 and child # 2 1 1Director shall provide LPA with a plan for care and supervision concerning the usage of restrooms by children in care. The plan should entail their process to ensure children are observed at all times. Director shall institute a plan to ensure children are supervised at all times while using the admitted the incident occurred; however, did not observe when child # 1 initiated the inappropriate contact with child # 2. Staff must provide supervision at all times. This is a Type A deficiency and it poses an immediate risk to the health and safety to the children in care. An immediate civil penalty in the amount of $500 dollars was assessed as of result of the absence of supervision. 8 restroom and provide training to all Staff concerning the new routine to prevent similar incidents reoccurring in the future. The plan must be submitted to the department no later than 1/19/2023.
