No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This was not evidenced by: 1 retrain lead teachers on name to face, provide resources for teachers to read and review. Director to provide signed written letter to LPA as proof of resources given to teachers. Within a week- additional training will follow. LPA found through the admission of staff a child was left unsupervised for a prolonged amount of time, in a classroom, sleeping, while staff and children were outside during recess. 8

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2), 1 Director will speak to staff and submit a signed letter to LPA explaining the understanding of what reporting requirements and the time frames in place to report incidents. Any unusual incident or child absence that threatens the physical or emotional health or safety of any child to the Department within seven days following the occurrence of such event. All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request and be approved for a transfer of a criminal record exemption, as specified in Section 101170.1(r), 1 Director will obtain criminal transfer exemption, and the teacher will not return to work until an exemption is obtained. LPA will conduct a follow up visit to confirm the teacher is not present within the facility. unless, upon request for a transfer, the Department permits the individual to be employed, reside or be present at the facility. the unusual incident reported on 10/07/2021 by assistant director 1 Director conducted a staff training on 10.11.21 to discuss personal rights. Director states they will have ongoing trainings to cover similar topics. Staff #1 was suspended immediately after the incident and they do not intend to bring her back. To correct the deficiency, director shall send to LPA a copy of the training agenda and a sign in sheet of which staff attended the training. This shall be done by 11.4.21. which stated that Staff #1 forcefully moved Child #1 against their will in a manner that was viewed by witnesses as inappropriate and aggressive. If not corrected, this poses an immediate risk to the health and safety of children in care.
