DEFICIENCY WAS CORRECTED IMMEDIATLEY. Director conducted a meeting with all the infant and toddler on 4/24/2025. Director provided meeting agenda and minute log and attendees. BAsed on the unusual incident report and direcot's account ot the incident, an infant was given the woring bottle. Infant did not surrfer from any adverse effects after ingesting wrong formul/milk.

Based on water testing results and interviews, facility tested over the Lead Exceedance level at 2 sink faucet measuring one at 6.9ppb and the other at 11.0 ppb. Director stated the faucet has been used to fill filtered Brita water pitchers and provided the water to children in the past. This poses an immediate/potential health, safety or personal rights risk to children in care.

101216 (i)(2) Personnel Requirements: Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not met as evidence by: 1 Director will associate staff to infant program and submit proof by POC date of 12/6/22. Proof will be maintained in staff file. A civil penalty was assessed. Based on observation and record review. Staff #1 is clear but not associated to the preschool program. This posses an potential risk to the health and safety of the children in care. 8
