Based on LPA observations and records review the licensee did not comply with the section cited above in cunts of four out of four . LPA reviewed 4 staff records and observed 4 staff members have expired certificates ,which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observarions and records review the licensee did not comply with the section cited above in 2 out of 2. LPA reviewed 2 infants files under 12 months and did not observe Individual Sleeping Plan (LIC 9227) , which poses/posed a potential health, safety or personal rights risk to persons in care.

Testing results with fractional ppb readings of 5.5 ppb or greater shall be rounded up to the nearest whole number...(1) A result with values of 5.5 or greater shall be deemed and Action Level Exceedance This requirement was not met as evidence by: Director will either remove the outlet or will flash or retest to comply with the requirements and will provide either pictures for he permanent removal or the retest results to teh LPA by POC date . LPA observed outlet B in the infant play yard . Facility immediatly covered the outlet with plastic bag and taped to make it is inaccessible . Pictures taken. 8
