Based on observation, interview and record review, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA Garcia observed Adriana Macias providing care and supervision to 5 children without meeting the minimum qualifications.
Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. Upon review of children's files, it was revealed that the LIC 627 Consent for Emergency Medical Treatment, was not on file.
The items specified above shall also be reported to the local health officer when appropriate... lists the following reportable communicable disease. This requirement was not met as evidence by: 1 Branch Director will send LIC624 UIR to Palmdale Regional Office no later than 07/14/25 regarding one diagnosis of hand, foot, and mouth disease. Child #1 was diagnosed with hand, foot, and mouth disease on 06/27/25 by child's doctor. Facility failed to notify department of this occurence, which poses a potential health and safety risk to children in care. 8
(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: 1 Staff #4 needs to be fingerprint cleared. Once Staff #4 is fingerprint cleared, Staff #4 needs to be associated to the facility. Risk Manager will notify he department once staff #4 is fingerprint cleared. Based on records review and on Guardian, staff #4 is not associated and is not fingerprint cleared which posed an immediate health and safety risks to children in care. 8
1596.7995. (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidence by: Risk Manager will send the immunization records of staff #4 via email. Based on records review, Staff #4 has missing immunization records which posed a potential health and safety risks to children in care. 8
Buildings and Grounds: The facility has rodents on the premises. According to Title 22 Regulations, the licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met as evidence by interviews conducted revealed rodents have been observed in the outdoor play area mostly at night. 1 The preschool will be closing for the last week of December 2021 for a deep cleaning. The facility has already taken corrective measures to ensure there are not rodents in the facility. A pest management service has been hired to clear and remove the rodents. This is a type B deficiency that if not corrected poses a potential risk to the health and safety of children in care. 8
