Based on observations , interview , records review , the licensee did not comply with the section cited above . During the tour LPA observed toddler room empty. Toured the preschool room and observed 15 napping children which 10 were preschoolers and 5 toddler children with 1 staff member . Per facility represantative napping combined due to the short staff , which poses an immediate health, safety or personal rights risk to persons in care.

On 07/10/23 from 8:20 AM to 11:30 AM LPA observed 3 toddlers were commingling with 3 preschoolers on toddler outdoor playarea and in toddler classroom. This poses an immediate health safety and personal right to the children in care.
