(1) No child(ren) shall be left without the supervision of a teacher at any time. 1 Director stated that the facility has completed re-training on 8/25/2025 for all staff regarding supervision and face to name policy to ensure the health and safety of children in care. The Director has submitted signatures of all staff members to the department. Based on interviews four (4) out of four (4) staff members stated that they have heard or witnessed C1 in the classroom without supervision for a period of eight and a half minutes. This poses an immediate health, safety or personal rights to children in care. 8

Based on interviews and record reviews, facility did not ensure that each child was accorded their personal rights, which poses an immediate risk to the health, safety and personal rights to the children in care. 8
