The Director agrees to submit the following POC: Meeting with staff regarding supervision and head count protocols. Staff will watch video regarding supervison. This requirement is not met as evidence by: Director reported on 5/16/2023 by telephone and written report on LIC 624 all children belonging to Destiny 2 class was not accounted for. C1 was left in the classroom unsupervised which poses an immediate health, safety or personal rights risk to persons in care. 8 Director will send written noticfication of understanding regarding supervision and send POC to LPA by 5/26/2023.

a)There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, 1 The director will submit a Plan of Correction to the department by 8/3/22 which will include how the staff will be trained to prevent supervising out of ratio. Indicating how the staff will remain within ratio at all times. This is evidenced by: Based on interviews from witnesses the staff have been observed supervising children out of ratio according to Title 22 Regulation. The ratio is 1 qualified staff person for every 12 children in care. This is a health and safety risk to the children in care. 8

Personal Rights- The licensee shall ensure that each child is accorded the following personal rights:(2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Upon arrival during the first inspection, LPA observed napping mats being utilized for the children on the premises. LPA interviewed the director today who stated they made changes after the parent made the complaint to her. LPA observed napping mats being utilized today. The staff failed to follow the napping regulation and violated the children's personal rights by having children nap with their heads down on the table which made it uncomfortable. 8
