; 1 By POC date, the facility representative stated the CCC will present a comprehensive training plan created and conducted by the Regional Director, of the for the care and supervision of children, to CCL for all permanent and temporary staff. The Regional Director will also provide a calendar of dates in which she will regularly be present at the facility to support the site, over the next 3 months, on a weekly basis. On 12/17/25 a child sustained unexplained injuries to the face while in care of the CCC, which staff interviews revealed to be due to lack of supervision which poses/posed a potential health, safety or personal rights risk to persons in care.

; 1 By the POC date, the Facility Representative stated they will provide CCL with a copy of the internal processes and procedures used to ensure the CCC is kept clean and sanitary at all times, in alignment with the cleaning and sanitation protocols outlined in their parent handbook. In addition, they will provide a copy of the schedule indicating the days and times when staff will conduct cleaning without the presence or supervision of children, as well as a list of the cleaning supplies used, accompanied by a recent invoice demonstrating that sufficient quantities are maintained for the center and documentation of the ordering process and the outside contracted cleaning service used nightly. Based on observation and interview the licensee did not comply with the section cited above, where LPA observed an unlatched approx. 2.5 ft tall gate opening to 2 sheds, with open doors, one shed contained children’s outdoor materials and items used daily and shed two with open doors contained 4 hedge trimmers, several bottles of weed killer, bleach, multiple cans of house/building paint and other solvents/agents labeled toxic. Aside from doors fully open/ajar, shed also did not have a lock present, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation and interview the licensee did not comply with the section cited above, LPA observed the outdoor activity space for preschool does not have any shade available to children. The CCC has a blue line across cement area that children are instructed not to cross, which is where the only shade available to children is located, under a building roof overhang outside of classroom 6, which poses/posed a potential health, safety or personal rights risk to persons in care.
; Licensee agress to submit proof of measures that will ensure compliance with meeting this regulation by POC date. LPA observed 4 large gopher holes that were located in the outdoor activity space, with mounds of burrowed dirt over 6 inches, spilling over onto bike path and other activity areas for the children, which poses/posed a potential health, safety or personal rights risk to persons in care. 8 Licensee agrees to submit proof of measures that will ensure compliance with meeting this regulation by POC date.
; 1 Licensee agrees to immediately ensure all children have access to toilet paper and provide photo evidence of a working toilet paper dispenser in good repair for all children’s toilet by POC date. LPA observed that 3/3 preschool classrooms in use did not have a toilet paper dispenser, where the wall next to the toilet had evidence of the removed dispenser only. 2/3 classrooms had available toilet paper for children to use located on an upper window ledge, approximately 5 foot high and not accessible by children, which poses/posed a potential health, safety or personal rights risk to persons in care.
; 1 By POC, Facility representative agrees that a Regional Administration staff member will submit to CCL, a written plan of strategy to ensure the regulation cited, CCR 101229(a)(1), is immediately met and provide the scheduled date and time of an in person staff meeting and training for all staff on site, which will be conducted by a Regional Administration staff member during the week of 4/28/25, covering the topics of the strategy plan submitted to CCL and the regulation cited. On 03/27/2025 a child sustained injuries requiring medical treatment, by staffs own admission, corroborated interviews and video evidence, staff failed to provide supervision at the time of the injury.

101229(a)(1) The licensee shall provide care and supervision as necessary to meet the childrens needs (1) no child shall be left without the supervision of a teacher at any time...supervision shall include visual observation. This requirement was not m:et as evidenced by 1 Leticia Gonzalez stated that potential blind spots in the outdoor activity area will be identified and discussed with all staff in a training on care and supervision and will provide a copy of the materials used during training where potential blind spots are discussed, with the sign in and out sheet for all staff, after the supervision training occurs no later than COB 02/21/25. Based on interviews, it was revealed a child had wandered away, unnoticed, and without supervision for a period of time between 1-5 minutes. This poses an immediate health and safety risk to the children in care. 8
Based on observation and interview, the licensee did not comply with the section cited above in where classroom number 6 bathroom, which included the toilet and ceiling, were in disrepair until todays date. Additionally, the fire alarm is not in proper working order, classroom #3 back door does not lock allowing access to outside alley of the CCC, Classroom #5 and # 2 front door handle are not in proper working condition, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, 3/3 preschool classrooms (Rooms 4, 5, 6) found to have chipping paint on indoor walls, cabinets chipped and in disrepair and childrens furnishings in disrepair.
During the tour of the facility, LPAs found that 8/10 nap mats used in classroom 4 were found to have ripped vinyl, sharp edges due to the rips in the vinyl and/or exposed foam in areas of the cracked and ripped vinyl
Based on observation and interview, the licensee did not comply with the section cited above in where 2/3 children’s toilet in the preschool facility were not in proper working condition and 3/3 classrooms in the preschool were found with to have visible dirt, grime and residue on the sinks, floors, walls and cabinets of the facility.
Based on observation and interview, the licensee did not comply with the section cited above in where the garden portion of the outdoor activity space contains a garden box with protruding nails, screws, rotted wood pieces, debris and a wooden bench in disrepair also with protruding nails and broken wood pieces which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interviews with 3/3 staff, interviews revealed child was left out on playground for at least 5 minutes. This poses an immediate health and safety risk to the children in care.
