; 1 By POC date, Facility Representative stated the CCC will provide evidence to CCL of the action taken immediately following the incident and the staff trainings on supervision that the CCC has since conducted in response to the lack of supervision occurring on 12/29/2025. On December 29, 2025 the CCC self-reported that an infant was left alone, without supervision, outside of the classroom for approximately two minutes, which poses/posed an immediate health, safety or personal rights risk to persons in care.
