Based on observation and interview, the licensee did not comply with the section cited above in 56 out of 56 persons in that LPA observed adult scissors on a counter in Room 5; a basket on a table containing several containers of sunscreen, scissors on a cubbie and a mop with an attachment containing cleaning solution in Room 7 which poses a potential health, safety or personal rights risk to persons in care.

Based on documentation and video footage, the licensee did not comply with the section cited above, as a child left through the playground door, and into the building unsupervised for approximately 1 to 2 minutes before the authorized representative noticed the child inside of the facility unsupervised, which poses an immediate health, safety, and personal rights risk to persons in care. 8 The Assistant Director will submit the POC to SDRO no later than COB on 10/02/2023.
