101229(a)(1) Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, This requirement is not met as evidence by 1 Per Principal they are now removing magnets from doors keeping doors lock. In the afternoon areas outside are condesed Prrincipal will conduct a staff meeting discussing licensing visit and will provide trainings on supervision with staff. Resources provided from the Technical Based on interviews the licensee did not comply in the section above in that children were unsupervised on 1/5/24 child went inside class, 1/17/24 two children were left unattended in bathroom & 1/17/24 child left classroom. This poses an immediate risk for the health and safety of children in care 8 Support Program will be provided to staff. Pricipal will submit meeting minutes and signing sheets of meeting.

This requirement was not met as 1 Principal will rotate staff accordingly to staff needs. Principal will provide proof of meeting with staff by POC date. evidence by: Based on interviewes S2 stated that S1 stepped out of the classroom leave S2 and S3 who are aides to provide care and supervision for a short period of time. This poses a potential health and safety risk to children in care. 8

This requirement was not met as evidence by: Based on interviews 1 Principal will review the usage of the steam lab to staff, reminding the staf to give children enough time to rest before offering the steam lab. S1 did not allow C1 to nap/rest and C1 was sent to the steam lab. This poses a potential health and safety risk to children in care. 8

Lead Teacher states only one person will be assigned to the restrooms and children will probably be going into the restrooms in the classrooms. LPA will be sent a statement from Lead Teacher. Director will include staff communication training. LPA will be sent agenda and staff sign ins. staff disclosures and LPA observation. Information was obtained disclosing that children are unsupervised in the outdoor restroom area when a staff member is not physically inside of the restroom. LPA also observed this to occur during this inspection. Staff are not visually observing children when they are in front of the restroom door facing the playground or on the steps leading to the restroom. This is an immediate risk to the health and safety of children in care.

Based on LPAs interview disclosures on 08/10/2022 from child #1 who states staff #3 and staff #7 were outside when child fell. Per Principal, she will provide training to all staff in regards to the responsibility for providing care and supervision to ensure staff use active supervision strategies and following Title 22 Regulations and will then submit the training agenda, materials and staff sign in attendance for the training and compliance plan to CCLD by or before 10/14/22 in order to clear this citation. LPAs interview disclosure confirmed by Principal that incident took place on 10/03/222, where children in care were left alone in classroom unsupervised by Staff #1. The second incident that occurred on 10/05/22, Child #1 walked out of the classroom interviews conducted by LPA determined that the Staff providing care and supervision in room 2-101 confirm that they did not notice when child #1 walked out of classroom. This poses an immediate health and safety risk to children in care.
