At approximately 2:28pm on 10/26/2021, LPA observed Staff 1 supervising 6 infants including 4 infants in the crib room and 2 infants in the activity 1 Licensee Chrissanthi Wickremesinghe agreed to maintain proper ratio at all times. Licensee agreed to rearrange staffing and submit an updated LIC500 Personnel Report with revised staff schedules along with a written statement describing how deficiency has been corrected by 10/27/2021. areas without any other staff members in the infant room. Both infants in the activity areas were awake at the time. Facility was operating out of ratio and that presented immediate risks to the infants' health and safety.
