Based on interviews, licensee spoke inappropriately, calling staff "stupid, lazy and worthelss" during facility hours of operation, in the presenece of children. This poses an immedicate health and safety risk to chidlren in care.
During interviews, it was disclosed that staff took the subject child into the office... 8 ...and closed the door. Later, the child came out and was seen crying. Interviews disclosed that the subject child had gotten yelled at by the staff. This is an immediate health and safety risk to children in care.
The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Based on observation, the licensee did not comply with the section cited above. 1 Immediately, facility staff has agreed to ensure there is no yelling in the presence of children. Licensee agrees to have a meeting with staff about personal rights of children. Submit agenda of training in personal rights as well as a written understanding. On or about Sept 8, 2022, E1 yelled at staff in the presence of children, which resulted in the children feeling sad, scared and wanting to go home after the verbal altercation between the two staff. During the facility tour, LPA's observed two boxes of powdered laundry detergent on the floor of the kitchen food prep area which was secured in a locked location. This poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights. This requirement was not met as 1 Facility director Patricia Schilling stated that the staff member has received verbal warning. Director agreed to provide training to all staff on Section 101223 Personal Rights as well as the facility's diapering policy and procedures. Proof of training with staff signatures will be submitted to the Department by the next business day on evidenced by: Staff admitted to doubling up the pull up instead of removing the pull up the child was wearing when changing a child. That violated the child's personal rights and presented immediate risks to the child's health and well-being. 11/15/2021.
At approximately 2:17pm on 10/26/2021, LPA observed 2 staff members supervising 26 children in the 1 Licensee Chrissanthi Wickremesinghe agreed to maintain proper teacher-child ratio at all times. Licensee agreed to rearrange staffing and submit an updated LIC500 Personnel Report with revised staff schedules along with a written statement describing how deficiency has been corrected by 10/27/2021. transitional kindergarten/kindergarten class. The children were having circle time with the staff members at the time. Facility was operating out of teacher-child ratio and that presented immediate risks to the children's health and safety.
