The licensee disclosed that on 11/30/2023, a child was taken to the TK classroom (which is part of preschool program), to nap. This poses an immediate Health, Safety, or Personal Rights risk to children in care. 8 Please note that this is a repeat violation/citation and therefore a civil penalty ($250.00) is being issued.
Based on the observation made by LPA upon entering a 11 children being left alone with no staff/teacher. This poses an immediate health, safety or personal rights risk to persons in care. Civil Penalty on $500.00 is being assessed.

Based on record review, the licensee did not comply with the section cited above as C1 and C2 were missing proof of completed LIC9224 that have been issued since March of 2023 which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above as LPAs observed a wooden play structure located in the outdoor play area in disrepair with peeling paint and loose wooden boards both on top of the ladder and attached to the foundation of the wooden play structure which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher-Child Ratio: A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. Based on observation, the licensee did not comply with the section cited above. 1 Staff returned a few minutes later to then comply with the regulation. Deficiency/citation was corrected during the visit. LPA Lopez and LPM Ross observed one staff supervising 16 children while another staff went on a restroom break. This poses an immediate health, safety or personal rights risk to persons in care. 8
Personnel Requirements: Request a transfer of a criminal record clearance as specified in Section 101170(f). Based on record review, the licensee did not comply with the section cited above. In reviewing staff associations, it was determined that the staff observed was not associated to the 1 Director agrees to associated the staff to the license/program to where the staff will be working in/for.
