Based on interview and observation, Licensee did not ensure that a cinder block wall that poses a tripping/falling hazard, was made inaccessible to children in care, which resulted in child injury requiring medical. This posed a potential risk to health, safety, and personal rights of child in care. 8 Information will be sent to LPA by POC date. Until the area is officially made safe, Area 5 will remain off limits during play time.
Based on record review and interview, the licensee did not comply with the section cited above ias 2 of 6 staff files, Staff #5-6, did not have a current Mandated Reporter Child Care Provider (AB1207) training complete, which poses a potential health, safety or personal rights risk to persons in care.
