Admission Agreements -The licensee shall comply with all terms and conditions set forth in the admission agreement. This requirement is not met as evidence by: Director stated that in the future the facility will have a parent meeting with the parents and possibly having the parent observe their child. Set up a behavior and prevention plan with the parents and have written documentations prior to termination. On 2/24/2023, licensee terminated a child's service without making every effort as stated in the facility's parent handbook prior to the immediate termination. This poses a potential health and safety risk to clients in care. 8 Director will submit a written plan of correction to LPA no later than 4/07/2023.

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidence by: 1 Director removed the two disrepair window screens during today's inspection. LPA Siao observed two broken window screens in the Big Room. This poses a potential health and safety risk to chidlren in care. 8

Outdoor Activity Space - The surface of the outdoor activity space shall be maintained: Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidence by: Director stated that there is a service order send out to the fence repairman. Director will look into putting rubber mulch or padding for the playground by 04/28/2023. LPA Siao observed the playground covering to be in disrepair as the green covering is torned and the grounds are uneven which is a tripping hazard. The movable mesh fence near room 3 is ripping. This poses a potential health and safety risk to children in care. 8

As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. This requirement is not met as evidence by: 1 Director will look into putting rubber mulch or padding for the playground by 04/28/2023. LPA observed the Liitle Tikes play structure is placed on top of green carpet and there is no cushioning around or under the play structure. This poses a potential health and safety risk to chidlren in care. 8

Based on the interviews conducted with staff/parents and LPA's observation of the facility's physical plant, it was determined that the facility was not safe due to the latch to the fence was broken, fence had holes that a child can fit through, holes in the play area that poses a tripping hazard and the barricades that were not sturdy and not anchored properly. This poses a potential risk to the health, safety and personal rights of children in care.
