Preschool children classroom is being commingled with a school age classroom to meet ratios. 1 Director, Patricia Green agrees to submit a written plan detailing how to ensure the preschool children and school age children will not be commingled outside of the waiver for the first and last hours of the day.

Based on interview and record review, the facility did not comply with the section cited above for the entire site and all classrooms. Upon inspection there was no documentation available for review which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the facility did not comply with the section cited above in one count out of six persons. Upon inspection of record review, immunizations records were not available for staff 1, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the facility did not comply with the section cited above in five out of six persons. Four staff had expired documentation certificates for mandated reporting and have not been updated, and S4 was missing which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff are commingling day care children.
