Each licensee...shall furnish to the Department reports as required...a report shall be made ...within the Department's next working day... a written report submitted to the Department within seven days.This requirement was not met as evidence by: 1 Judy Heffernan agrees review the reporting requirement technical assistance provided by LPA with Director Tricia Green. Based on interviews and records review, the facility did not report an incident regarding the facility's air conditioning not being on good repair on one occasion. This poses a potential risk to the health, safety and personal rights of the children in care. 8

C1 biting and scratching five children from 03/20/23 to 04/10/23.and has been caught trying to bite other children on numerous occasions. Per 1 Director, Patricia Green agrees to conduct a staff meeting on providing care and supervision and submit a copy of the intinerary of the supervision training with signatures to Community Care Licensing by 04/12/23. staff C1 has a history of biting and is agressive. This poses an immediate risk to the health and safety of the children.

Based on record review, the facility did not comply with the section cited above in which staff #2, #3 and #4 do not have immunization records available for review during this inspection wich poses a potential health, safety or personal rights risk to persons in care.

Based on observation the facility did not comply with the section cited above in which two infants did not have dates listed on their bottles inside the classroom refrigerator which poses a potential health, safety or personal rights risk to persons in care.

Based on record review the facility did not comply with the section cited above in which staff #3 does not have a current Mandated Reporter Certificate available for review during this inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
