Licensees shall maintain a lead value at or below the Action Level of 5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care. This requirement is not met as evidence by: 1 Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Action level. Based on record review of required lead testing, the facility had lead values of 5 or above on water outlets: A,B, and D from 12/2025 and 01/2026 retests. This is a potential health and safety risk to persons in care. 8 Facility will notify CDSS with completion within 30 days.

(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 Facility agrees to submit an agenda for the next meeting, scheduled for 06/25/24, involving knowing the "Responsibilities for Providing Care and Supervision". Director will also submit proof of all staff attending. Based on facility admission, a child was left without the supervision, including visual observation of a staff. This poses a potential risk to the health and safety of children in care. 8
