The Director stated that all staff will review Title 22 regulations in the areas of personal rights and responsibility for providing care and supervision. Director stated that on 8/1/25 there will be a staff development day where the staff will review Title 22 regulations regarding personal rights and responsibility for providing care and C1 sustained an injury on their back that staff was unable to determine how the injury occurred. The injury resembled fingerprint marks. This is an immediate risk to the health and safety of children in care. 8 supervision. Staff will also review writing appropriate incident reports and review several supervision scenarios including indoors/outdoors. Director will submit a copy of the training roster for the 8/1/25 meeting. Director will submit written statement to CCL outlining the above by plan of correction date- 7/24/25.

Director stated that she is going to review the parent handbook and do the needful corrections to the parent handbook, including the review of enrollment application. This review will include termination of services. Director will provide an updated copy of the parent handbook to parents and to CCL by Center Management staff terminated enrollment of C1 which is not in accordance with the parent handbook. This is a potential risk the health and safety of children in care. Plan of Correction Date- 8/22/25.

Surveilane footage shows S1 pointing their finger in C1's face multiple times, pushing C1 to an area of the classroom and aggressively grabbing C1 by the arm to redirect and reprimand them. This was seen more than four times and is an immediate risk to the health and safety of children in care. 8 occurred, which will include classroom expectations and teacher to child interactions. Director will submit a written statement by plan of correction date confirming when the above meetings will occur.

Director stated they will request a waiver to allow for commingling for the last 15 minutes of the day for preschool and school age children. During the course of the investigation LPA learned school age children and preschool children were being commingled. This poses a potential health and safety risk to children in care.
