Facility self reported an incident that ocurred on 7/22/26 where child 1 (C1) was unintentionally left alone in a napping room. This is an immediate risk to the children in care. 8 Director will provide a written statement confirming training as well as submit a detailed outline of the new transition protocol. Plan of Correction date is 7/30/26. Civil Penalty of $500.00 will be assessed.
