Based on interviews and record reviews, a lack of supervision occurred for Child #1 (C1) during a 10/30/24 self-reported incident at the facility when she left her classroom unnoticed and was gone for a reported twelve minutes before staff was made aware of her absence which posed an immediate Health and Safety risk to the child in care. 8 The plan of correction does not exclude the Department from taking any additional action it may deem necessary regarding the incident.
