Based on staff#3 and staff#4 interviews, both were present on the sand outdoor playarea on 02/23/26, but they did not see how child#1 fell down and hit forehead on the tree log and sustained big bruise on forehead. … 1 Per director, all staff will be reminded to call parents if the incident is serious. A copy of message will be submitted to LPA by 04/30/26. Per staff#1, the incident occurred on 02/23/26 at 9:17:26 AM, a message was sent to parent#1 at 9:33 AM via procare, but school did not attempt to give a call to parent#1 after noticing the seriousness of the big bruise on child#1’s forehead. This poses a potential health, safety, and personal right risk to the children in care.

… 1 Per director, staff will be reminded if the injury is serious and it is needed to be iced, staff bring the child to the office or put an additional effort to apply ice on the child's injury. A copy of message will be sent to LPA by 04/30/26. Per staff#3 and staff#4, ice was applied on child#1 forehead after fall, they went inside that classroom at 9:30 AM and child#1 did not like the ice and staff did not apply ice on child#1’s forehead till was picked up by parent#1 at 10:17AM. This poses a potential health, safety, and personal right risks to the children in care.

… 1 Per director, tree logs were removed right away after the incident. LPA did not observe any tree logs on the front sand box. Per staff interviews, there were tree logs on the sand area in a way to stand on and toddlers and younger preschoolers were playing in that area. This poses a potential health, safety and personal right risks to the children in care. Per director, the toddler program was approved by the department while tree logs were on the front sand box for toddler and younger preschoolers use.
