This incident presents an immediate risk to the health, safety, and personal rights of children in care 8 classroom staff are trained accordingly. Documentation of staff training, updated procedures, and verification logs must be submitted to LPA by 10/8/25. The Site Supervisor is responsible for ongoing monitoring and enforcement of the corrective actions as outlined.
