review of video showing S1 looking away from C1 while C1 was ontop of changing 1 Director will write out changing diaper procedure and have teacher sign off and each teacher will demonstrate ability to follow correct procedure and send documentation to LPA by plan of correction date. table at which time C1 rolled off and fell. This poses a potential risk to the health and safety risk to children in care.

Director implemented keeping a binder with each child's Infant sleeping plan in the infant room in order to keep checks and balances; C1 has disenrolled and director is unable to obtain infant sleeping plan. LPA viewed citation corrected during visit.. Civil penalty stopped Review of 1 of 2 Children's files did not have an infant sleeping plan in file(C1). This is a potential risk to children in care; Director state she gave plan to parent and it was not returned ; C1 admitted 050222. Civil penalty will be issued
