This requirement was not met evidenced by: 1 Director states that they will submit a detailed and modified plan to ensure Teacher-Child Ratio are being met at all times Verification will be sent to LPA Castro via email (alma.castro@dss.ca.gov) by the due date. Based on observations and interviews during 06/09/2026 visit and record review, S2 (who was not qualified as a teacher) was providing care and supervision for six school age children in the school age classroom by themselves prior to 8:50ams, which posed an immediate risk to the health, safety and personal rights of the children in care. 8

This requirement is not met evidenced by: 1 During the visit, Director allowed the school age children to return to play after speaking with LPM Tran. Director stated she would review personal rights regulation with facility staff and submit proof of training with staff to LPA by due date Based on observation and interview, six school-age children were asked to sit down by the facility fence, according to the Director, for their safety, which posed an immediate risk to the personal rights of the children in care. 8
