Child Care Center failing Director to submit a plan to ensure proper supervision is maintained and children in care are routinely checked for cleanliness. Further, Director to create a plan whereby parents are informed of incidents concerning children in care. to properly change the diaper of C1,failing to clean the mucus from the face of C1 and failing to afford proper supervision to C1 to account for instances of C1 being bitten and having facial area scratched. This poses a potential health, safety or personal rights risks to persons in care.
A number of children in care were checked in access of 15 minutes and a number of entries were void of the intials of the staff member who completed the safety check. This posses an immediate health and safety risk to the children in care.
Conduct Inimical- Licensee shall not engage in conduct that is harmful to the health or safety of an individual in the facility; This requirement is not met as evidence by: 1 Licensee to submit a plan of correction by 12/29/21, noting how to ensure proper safety checks are completed and documented by staff members. A Civil Penalty of $250.00 was assessed during today's inspection as the deficiency is a repeat violation. A review of a sampling of sleep logs and sign in/out documentation reveal safety checks being completed and comments about physical state being made after children were checked out of the CCC. This posses an immediate health and safety risk to the children in care. 8
During LPAs observed that S1 providing supervion for 8 toddlers. This posses an immediate health and safety risk to the children in care. 1 Licesee will submit a plan of correction by 12/29/21 to ensure that staff-children ratios are being followed at all times. A Civil Penalty of $250.00 was assessed during today's inspection as the deficiency is a repeat violation.
C19 was able to access and turn on CCC's main water main, causing 1 Licesee will submit a plan of correction by 12/29/21 to ensure the main water main is not accessable to children in care. a stream of water to force C19 to the ground and be soaked with water in 49 degrees temperture. Additionally children were observed on site with runny noses while coughing and sneezing. Moreover, S1 did not wear gloves when changing a child in care. This posses an immediate health and safety risk to the children in care.
Infant, School age and preschool aged children were observed in the outdoor playground. This posses an immediate health and safety risk to the children in care. 1 Licesee will submit a plan of correction by 12/29/21 to ensure the children of different age groups are not commingling.
LPAs observed one teacher providing supervision for five infants. Director will submit a plan of correction by 11/16/21 to ensure that staff-children ratios are being followed at all times. A $250 Civil Penalty was assessed during today's inspection This posses an immediate health and safety risk to the children in care. 8 Under Appeal
Conduct Inimical- Licensee shall not engage in conduct that is harmful to the health or safety of an individual in the facility; licensee shall not fail to comply with health officer orders or government agency requirements that creates a significant health and safety risk pursuant to CDSS statutes and regulations. This requirement is not met as evidence by: 1 Licensee will immediately adhere to the Guidance for Child Care Providers and Programs and Health Officer Order # 2021-10.6 County of Santa Barbara and ensure all teaching staff appropriately wear face mask covering nose and mouth On 11/18/21,S1 failed to wear face covering inside of the CCC while supervising children in care. This posses an immediate health and safety risk to the children in care. 8 Licensee to submit a written statement of the adherence to Guidance for Child Care Providers and Programs and Health Officer Order # 2021-10.6 no later than 11/19/2021.
Based on LPA's review of Facility Personnel Report, it was observed that Tim Miller is not associated with Small Wonders in Santa Maria, CA. This poses an immediate health and safety risk to the children in care. 8 Under Appeal
Director will submit a plan of correction by 11/16/21 to ensure that staff-children ratios are being followed at all times. During LPA inspection of facility it was observed that S7 was caring for five (5) infants. At 12:11 PM, LPA observed S9 was caring for nine (9) infants. This posses an immediate health and safety risk to the children in care.
Licensee will immediately adhere to the Santa Barbara Public Health Officer Order No. 2020-10.1 and Health and Safety Code Section 1596.885. Licensee to submit a written statement of the adherence to PHO order and Health and Safety Code to CCLD no later than 9/22/2021. On 9/21/2021, LPAs observed S1, S2, S3 & S4 not to be properly wearing mask, failing to cover the mouth and nose, which displayed willful disregard of the ordinance of the Santa Barbara County Public Health Officer Order No. 2020-10.1. ordering individuals in the County to wear face coverings when in high-risk situations that may contribute to the spread of COVID-19. This poses an immediate risk to health and safety of children in care.
Based on the LPA's observation/record review revealed Consent for Medical 1 Front desk aid acknowledged not having Consent for Emergency Medical Treatment form in C5, C6, C8, C9 C10, C11, C12, C13, C14, C15, C16, and C17 files. Director will provide verification of form completed by parent on or before 09/28/2021. Treatment not to be in Children's files for Licensee acknowledged not having Consent for Emergency Medical Treatment form in C5, C6, C8, C9 C10, C11, C12, C13, C14, C15, C16, and C17 files. This poses a potential risk to the health, & safety to children in care. Under Appeal
Based on the LPA's observation/record review revealed S1, S2, & S3 did not meet the requirements for infant toddler teacher. 1 Director will provide verification of infant units taken by S1, S2 & S3 by on or before 09/28/2021 This poses a potential risk to the health, & safety to children in care.
