TSO served on 12/28/21. Licensee will immediately adhere to the Gudiance for Child Care Providers and Programs and will use gloves while preparing and serving food. Licensee to submit a written statement of the adherence to Guiidance for Child Care Providers and Progams no later than 12/29/2021. On 12/28/21, Facility did not ensure the personal rights of persons in care to safe and healthful accommodations and engaged in conduct inimical to the health, welfare and safety of person in care, in that LPAs observed S1 did not use gloves for tasks such as serving food as stated in Guidance for Child Care Providers 8 and Programs effective 6/29/21.
(a) In addition to Section 101238.2, the following shall apply: (b)... outdoor activity space provided for school-age...separated from space provided for other child care center children. (1) Physical separation between school-age and other child care center children shall not apply when a planned activity is being conducted. 1 TSO served on 12/28/21. Licensee will ensure that age groups do not commingle and will submit plan of correction to LPA Velazquez by 12/29/21. Plan of Correction will detail steps the Director will take to ensure commingling of ages does not occur at all. On 12/28/21, at 3:33 PM, LPA observed 10 children outdoors. After reviewing facility roster it was noted that 9 school-age children were commingling with a two (2) year old. At 4:40 PM, LPA observed 23 children outdoors of mixed age groups. 8
Director agrees to create a plan of correction detailing how she will remain in complaince with Ratios at all times in the facility and will submit to the Department by 11/19/21. At 10:55 AM, LPAs observed S1 and S2 providing care and supervision to 27 preschool age children. This is an immediate risk to the health and safety of children in care. 8 CCR
the following: 1 Director to remove objects (i.e. chair, box and plastic bubble wrap) from in front of the CCC's rear exit door. Photograrph of the correction to be forwarded to CCLD by 11/19/21. LPA observed the child care center exit door blocked by chair, box and plastic bubble wrap. This poses a potential risk to health, safety or personnel rights of persons in care.
the following: 1 Director will submit a plan of correction detailing how Director will ensure that child care center lighting fixtures are in good repair and working at all times. Plan of correction to be submitted by 11/25/21. LPA observed the child care center has some light fixtures that do not work. LPAs also observed fixture covering the light in the children bathroom was not closed and left hanging. This poses a potential risk to health, safety or personnel rights of persons in care.
Conduct Inimical- Licensee shall not engage in conduct that is harmful to the health or safety of an individual in the facility;licensee shall not fail to comply with health officer orders or government agency requirements that creates a significant health and safety risk pursuant to CDSS statutes and regulations. This requirement is not met as evidence by: 1 Licensee will immediately adhere to the Gudiance for Child Care Providers and Programs and Health Officer Order # 2021-10.6 County of Santa Barbara and ensure all teaching staff appropriately wear face mask covering nose and mouth. On 11/15/21, Director did not ensure the personal rights of persons in care to safe and healthful accommodations and engaged in conduct inimical to the health, welfare and safety of person in care. S4 was observed not wearing a face mask covering nose and mouth. S6 and eighteen (18) preschooler were observed without face masks. 8 Licensee to submit a written statement of the adherence to Guiidance for Child Care Providers and Progams and Health Officer Order # 2021-10.6 no later than 11/16//2021.
101238 Buildings and Grounds: (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidence by: 1 Director will submit a plan of correction ensuring the safety and well-being of children and will take meausres to keep the center free of insects by 9/23/21. While conducting facility tour, LPAs observed that the preschool sleeping mats had ants crawling on the sleeping mats. This is an immediate risk to the health and safety of children in care. 8 Under Appeal
