on 01/11/24, Child #1 pushed Child #2. Staff #1 disclosed grabbing Child #1 by the wrist/arm due to pushing Child #2. placed them in a seated position. Child #1 began to cry, and Staff #1 began to rub their left arm/hand. 1 Assistant site supervisor stated she will submit a written plan of correction indicating the steps she will take to avoid similar incidents in the future. The plan of correction shall be submitted to the Department no later than 06/14/24. Additionally the facility will outsource training concerning Personal Rights Child #1 was taken to the doctor for evaluation and was diagnosed with an unspecified fracture of the lower end of the left radius. This is a Type A deficiency that poses an immediate risk to the health and safety of children in care.
