Based on observation, the licensee did not comply with the section cited above. While touring the outdoor activity space at 9:41 a.m., LPA's observed broken wood and chipped paint on the infant gate which poses/posed a potential health, safety or personal rights risk to persons in care.

During file reviews, LPA's observed there were no documentation for 15 minute checks on infants during nap times. Staff stated they were not documenting infant sleep every 15 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.
