Based on interviews, records review, and observations, the licensee did not meet the requirement to not accept children with symptoms of illness. 1 The director stated that she would request a doctor clearance note when children arrive with symptoms or develop symptoms while in care before accepting the child. If a parent doesn't bring a doctor's note, the child will not be accepted. In addition, the licensee failed to notify the child’s authorized representative as required by 101226(a). The LPA observed children with obvious symptoms and S6 said the parents had not ben notified. This poses an immediate risk to the health and safety of children in care. 8 The director will email all the parents informing them that if a child is ill or has symptoms they will not be accepted unless they present a doctors note stating the child can be in care at the facility. The director will send the LPA a copy of the email by 7/1/2025.

Based on interviews and records review, 1 The director will review the e-learning videos on the cdss website at https://ccld.childcarevideos.org/ and review the following trainings: the licensee did not meet the requirement to report an unusual incident involving a child who needed emergency medical treatment and was hospitalized for 2 days or more. S1, S4, and S6 disclosed that the incident had happened. S6 stated the incident was not reported to licensing. This poses a potential risk to the health and safety of children in care. Child Care Reporting Requirements Health and Safety Training. The director will send a declaration stating she has reviewed the videos to the LPA by the due date.

Based on records review and interviews with the director, the facility does not have a sign in and sign out sheet that meets the requirement. The sing in/out sheets observed and obtained from the director were incomplete and/ or incorrect. 8 The director will email the LPA with proof of correction including a declaratin the issue has been fixed and copies of sign in sign out sheets that are up to regulation. This will be completed by July 30, 2025.

Based on records review, the licensee did not comply with the section cited above in 3 of 5 staff, which poses/posed a potential health, safety or personal rights risk to persons in care. Two staff did not have a LIC503 or qeuivalent. One staff who had a complete an LIC503 did not have proof of tuberculosis testing.

Based on records review, the licensee did not comply with the section cited above in 4 of 5 staff, which poses/posed a potential health, safety or personal rights risk to persons in care. LPA checked 5 staff files for compliance. Four of the staff did not have a current Mandated Reporter certificate. The director stated they would complete a new one soon.

Infant Care Personal Services §101428(b)
(b) The infant shall be kept clean and dry at all times.(2) Each infant's clothing and diapers shall be changed as often as necessary to ensure that the infant is clean and dry at all times. This requirement was not met as evidence by records review and statements. Director agrees to provide a training to the staff on Diaper changing and the regulation addressed. Director stated that she will provided a training agenda and role sheet of infant staff provided the training. Each staff will sign the role sheet. This training will be conducted and provided to LPA via email to stella.gutierrez@dss.ca.gov On 06/09/2022 records reviewed revealed that the child in question was not provided a diaper change for a long period of time. Statement received by a staff stated that the children are not provided a diaper change when napping so the child is not interrupted. This poses a risk. 8 by the specified date of 10/07/2022
