Based on observed, the licensee did not comply with the section cited above in that LPA observed a community sandbox that is used by both school age and preschool ages. LPA observed that sandbox had multiple animal fecal inside. LPA observed that sandbox did have a cover, yet sandbox was not covered when not in used. LPA discussed with Staff #5 best practices, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA was informed that Teacher aides do help out, but that the facility does not have a qualified school age teacher, which poses/posed a potential health, safety or personal rights risk to persons in care.
