The licensee was made aware of an alleged incident involving staff-children interaction on 05/02/2024 and the licensee did not notify the department of the unusual incident within 24 hours, which poses a 8 potential risk to the personal rights of persons in care.

101700.3 (b)(1): A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidence by: 1 POC: Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Based on records review of required lead testing results on 03/03/2023, the facility had lead values of 5.5 or above on water Outlet C, 12.90 ppb and Outlet D, 19.70 ppb, which are located in the kitchen and utilized for consumption & food prep. This is a potential health and safety risk to persons in care. 8 Facility will notify CDSS with completion within 30 days.

Based on record review, the facility did comply w/ith written directives outlined in PIN 21-21-CCP and failed to have 3 kitchen sink faucet water sources tested for safe lead levels of 5.5 ppb or below, which is a potential risk to the health and safety of daycare children.

Based on records review and interview disclosure, the licensee did not comply with the section cited above in which for the first time while in care at the facility, a child was left in their diaper for approximately three hours which caused the child to sustain a diaper rash. This poses an immediate health, safety, and personal rights risk to children in care. 8 and sign in sheet can be submitted by 11/18/2021. 101428 Infant Care Personal Services regulation was provided to Director Wendy Leath during this inspection.
