Director states she will have a staff training in regards to supervision. LPA will be sent agenda and staff sign in sheet by 09/09/21. LPA observed a staff write up with the description of the incident which occurred. Disclosures were made in regards to the incident. LPA found that a child was left unsupervised on the upstairs playground on 10/09/18 for approximately 2 minutes when a staff from an upstairs classroom found the child and brought them back to the teachers. This was an immediate risk to the health and safety of the child in care.

LPA observing that an incident which occurred at the facility on 10/09/18 was never reported to the Department. This is a potential risk to the health and safety of children in care.

Director states they are scheduled for training. LPA will be sent POC once training is completed. POC due 09/03/21. staff file review. LPA observed that there is not a staff present at all times with current Pediatric First Aid & CPR, This is a potential risk to the health and safety of children in care.

LPA observed a paper dispenser/trash can that is attached to the wall that is broken; the door does not close. LPA also observed chipping paint in multiple classrooms which include Grown Up Blvd Room, Kinder Crossing and Tumbling Avenue. The disrepair's are a potential risk to the health and safety of children in care. Director states there is a work order for the painting and the dispenser will get fixed.

staff file review. LPA observed that Staff 2 through 5, do not have a health screening on file. This is a potential risk to the health and safety of children in care. 1 Director states all staff will have the health screening completed. LPA will be sent proof of corrections by 09/06/21.
