Although licensee did test for lead, LPA could not find the report on the California State Water Resources Board. Licensee did not follow written directives in PIN21-21-CCP.

LPA did not observe a Emergency Disaster Plan posted in the facility. Director stated she took it down and had it on her persons to work on it and was not present at the facilty during the inspection to post it.

Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA Garcia did not locate proof of complete immunization record in 2 teacher files reviewed. Director stated, she will ensure the teachers submit their updated records and keep on file.

Responsibility for providing care and supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without supervision of a teacher at any time,...supervision shall include visual observation. The requirement was not met as evidence by: 1 Licensee already has the finger pinchers at the doors. They will have staff check every morning to ensure finger pinchers are on all doors. Licensee trained staff to never leave children unattended under any circumstance. Based on interviews child #1 obtained a finger injury due to classroom door closing on child #1s finger and child #1 needing medical attention that resulted in child #1 obtaining a fracture on child's finger. Staff #1 was not present during the time of the incident that occurred to child #1. This poses an immediate health and safety risk to children in care. 8
