Facility will conduct a personal rights training to all staff, which will also include the CCLD video on Personal Rights and appropriate communication around children. https://ccld.childcarevideos.org/ . Facility will send LPA staff signatures showing their attendance and understanding of the meeting. Interviews revealed that staff #1 held on a child's chins and proceeded to tell him "Para Para" in harsh and intimidating tone of voice. This pose a potential health, safety, or personal rights risk to children in care.

Facility stated they are no longer using the outlet, but the faucet has been replaced. Facility will update LPA with results once retesting takes place. Lead test results reported to licensing showed a fountain in the facility with lead levels exceeding 5 parts per billion (ppb). This poses a potential risk to the health and safety of children in care.
