Based on interview and record review, the licensee did not comply with the section cited above in one out of six staff files reviewed which did not have proof of tuberculosis test or health screening, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observation and staff members disclosure that the location provides care and supervision to children from ages 5 to 12 years, which is outside of the limitation of their licensed capacity. This poses an immediate Health and Safety risk to children in care. 8 LPA informed the representative that an application (LIC 200A) must be submitted within 10 days along with an updated Facility Skecth.
