No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidence by: 1 Per Director a meeting will be conducted discussing findings staff will watch the video "Supervising Children in Child Care Centers" located at ccld.childcarevideos.org. Sign in sheet, meeting minutes and supervision plan will be submitted to LPA 11/21/2025. Based on interviews conducted, the licensee did not comply with the section cited above as Per 3 staff incident causing child # 1's eye injury was not obsered which poses/posed an immediate health, safety or personal rights risk to persons in care. 8

Per center director a copy of the training agenda and a sign in sheet will be provided to LPA by POC date 10/06/2025. C1 was unsupervised by staff members in the classroom, resulting in an immediate risk to the child's health and safety. 8
Per center director mopping with water alone during nap time will be done, and a staff training will be held on when and how to properly handle cleaning solutions. A copy of the training agenda and a sign in sheet will be provided to LPA by POC date 10/06/2025. Staff use cleaning solutions to mop classroom floors while children nap on napping equipment, on the floor.
Based on interviews conducted, the licensee did not comply with the section cited above as 5 staff disclosed children have been out of visual supervision of staff, which poses/posed an immediate health, safety or personal rights risk to persons in care. 8 Director stated the facility will install gates in the Pre-K and Preschool Classrooms at the end of the classrooms that lead to the bathroom.
Based on interview & record review, the licensee did not comply with the section cited above as CSR shows facility out of ratio on 10/1/24 & 10/2/24 & 3 staff disclosed facility has been out of ratio, which poses/posed an immediate health, safety or personal rights risk to persons in care. 8 Facility will also submit a written plan to LPA for maintaining ratio at the facility throughout the day, including the morning hours, days where the facility has staff shortages and staff leave the facility to make bus runs.
Based on interviews, the licensee did not comply with the section cited above as staff disclosed the childrens bathroom has been observed with urine on the floor and smears of poop on the ground, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interviews and observation, the licensee did not comply with the section cited above as staff are microwaving children's food in styrofoam bowls which are not designed to be microwaved, which poses/posed a potential health, safety or personal rights risk to persons in care. 8 not microwave safe. On 12/5/24, Director provided LPA with a copy of the message given to parents via the KinderCare app asking them to send their children's food in microwave safe containers.
Based on interviews conducted, the licensee did not comply with the section cited above as 3 staff disclosed children have eaten other children’s food without parent consent, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interviews the licensee did not comply with the section cited above as staff disclosed bedding was stored together & handed out to children without washing & at times not washed weekly, which poses/posed a potential health, safety or personal rights risk to persons in care.
