The remaining children left the facility at 4:41pm (1:3), 4:58pm (1:2) and 5:06pm (1:1) and 5:20pm (1:0). The facility was back in compliance at 5:06pm when Staff Person #1 was providing care for one (1) child in the facility.

The record for Staff Person #2 does not contain an initial health assessment with the results of initial tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Facility Person #2 has not completed the required one-hour Pennsylvania health and safety update 2022 by December 30, 2022.

Staff Person #1 has not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated 12/11/2018.

The director has not completed a written evaluation of staff person #1 on a regular basis, a minimum of one evaluation every 12 months. This is evidenced by the most recent staff evaluation contained in staff person #1's record being dated 1/4/2021.
