The Director or designated staff person who is responsible for compliance with this chapter did not ensure the requirements under section 1016(c) of the act (62 P.S. § 1016(c) are being met. This is evidenced by the fire detection system being tested on 10/11/2024 and not again until 11/22/2024, which exceeds the 30-day requirement.

Center’s plan of correction (implemented, Dec 11, 2024): “Monthly inspection form has been created for branch maintenance to complete. The fire alarm system will be manually pulled and checked each month, not to exceed 30 days.”

It was observed on 11/27/2023 in the Activity Center room there was a hole in the wall at the bottom of the chalk board frame and the baseboard was coming away from the wall. (CORRECTED ON SITE)

Center’s plan of correction (implemented, Jan 19, 2024): “During the inspection the hole in the wall was covered up with tape and the baseboard was reattached to the wall with tape until repairs can be made.”
