Staff #1was hired provisionally and did not have a disclosure statement, a PA State Police clearance, PA Child Abuse clearances, FBI clearance and a NSOR clearance on file and was working in direct care with children since hire (See LIS Code Sheet for DOH).
The inspector observed crackers that were identified as being served to children next to one another on the same shelf as a bottle of sanitizer solution in the toddler room.
The inspector found that the facility did not complete a fire drill every 60 calendar days as evidenced by the previous drill being conducted on 9/3/25 and the most recent held on 11/26/26.
A renewal inspection occurred at the facility. A review of staff files was conducted. Staff person #1 has not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): health and safety topics. Staff #1 completed this training on 4/3/24.
Staff interviews indicated the following incidents occurred at the facility. On 8/21/23, Staff # 1 told Child # 1 that the child needed to lay down and if Child # 1 did not lay down Staff # 1 would hold the child down. Staff # 1 then forced Child # 1 to lay down by forcefully pulling his legs out from under him (he was laying on his stomach with his knees scrunched up under him), turning Child # 1 so Child # 1 was laying on his back, and then pushing Child # 1 down on his back and holding Child # 1 there with their forearm. Staff # 1 held Child # 1 in that position for approximately 30-45 seconds. On 8/22/23, Staff # 1 grabbed a child by the arm in an angry manner and drug the child to a center. Staff interviewed said the child was upset and staff heard the child say to Staff # 1; "you can NOT do that to a child." Staff # 1 then responded; "Well the child needs to listen."
Staff interviews indicated that on several occasions, Staff # 1 has used harsh and demeaning language towards children. Staff # 1 also received a write-up on 5/11/23, due to this staff not using appropriate tones and disrespectful language towards children. Staff interviews also indicated that, Staff # 1 would treat Child # 1 different than other children in the way Staff # 1 spoke to the child and the actions of Staff # 1 towards Child # 1.
A renewal inspection occurred at the facility. A physical site inspection was conducted. Staff #2 was viewed diaper changing. Staff #2 did not wash their hands or the child's hands after the diaper change.
A renewal inspection occurred at the facility. A review of the staff files was conducted. Staff person #1, #3, #4, #5, #6, #7, and#8 did not complete the required one-hour 2022 update to the health and safety training by 12/30/2022. These staff completed the training on: Staff #1 - 2/26/23 Staff #3 - 1/10/23 Staff #4 - 2/10/23 Staff #5 - 1/30/23 Staff #6 - 2/10/23 Staff #7 - 1/31/23 Staff #8 - 2/5/23
“Core safety” marks citations about supervision and staff ratios, discipline and caregiver conduct, background checks, child protection and safe sleep, the topics our score weights most. Pennsylvania’s records don’t grade violations by severity. “Follow-up” marks a violation re-cited at a follow-up visit, counted once. How scores work.
Complaint investigations and reported incidents
Pennsylvania’s search lists complaint investigations in which the state found at least one violation (“verified complaints”). It does not say what a complaint alleged, or whether every violation relates to it. Incident investigations look into something that happened at the center; self-reported violations are ones the center reported to the state itself.
