Staff member number 6 did not have current Emergency Plan Training on file during the Renewal Inspection. The Emergency Training on file for this staff member was last done on 02/27/2023.

During the Renewal Inspection, 15 toddlers were observed in the Indoor Play Space at one time when capacity for this room - for toddlers is calculated as 6 at one time.

Staff members numbers 1,2,3 and 5 did not have current Mandated Reporter Training on file during the Renewal Inspection. Staff member number 2 did not have Health and Safety Training on file.

Staff member number 4 did not have staff evaluation on file during the Renewal Inspection and has hire date of 01/01/2023.

There was peeling paint observed on the stairs leading to the second floor and in areas of the hallway at the time of inspection on 2/21/24.

In the facility restroom on the second level there was a piece of wood with exposed screws and bolts that could pose a hazard to the children. There were also exposed bolts and screws on the door in the stall (toilet area) in the children's bathroom.

There was no written proof of a health assessment in the file for facility child #1.

At the time of inspection on 2/12/24, there was no written proof of training in the file for staff #1-#3 as required by regulation.

Observed second floor open space with no first aid kit.

Observed all classrooms on second floor missing daily schedule posting.

Observed all bathrooms with no handwashing sign posted.

Observed art room with no schedule posted.
