It was observed that the red and yellow play structure on the preschool playground was not in good repair. The bottom of the yellow slide was not attached on the left side; the bridge had multiple weak points and sharp metal edges; there were multiple spots of rust causing rough edges.
During naptime, in the Younger Preschool Room, Staff Person #1 identified 5 children in their primary care group and Staff Person #2 identified 3 children in their primary care group. There were 2 classes combined for naptime with a total of 14 children. Neither staff person included the other staff person's (on break) 6 children as part of their primary care groups.
A Medication Log documented that Child #1's medication was administered on 1/22/26, however the staff person had not included the time that the medication was administered.
Children in the Prekdg class were not regularly taken outdoors daily. Staff indicated that they take children out in the afternoons, but not always in the mornings. While children go to the gym some mornings for gross motor play, there are many children who are picked up before children go outside to play on the playground (4:15 or 4:30), so those children do not go outdoors daily, weather permitting. Class schedules include a morning and an afternoon outdoor play time.
Two Magic Erasers were located by the paper towels that the children use for handwashing. They were not labeled and were accessible to the children in the School Age Room/Gym.
Several hexagon rubber surfacing pieces on the Toddler Playground under the climber and at the entryway, were loose or missing.
A weighted blanket is used for Child #1 at naptime. The parent provided the weighted blanket and gave written permission to use it, however, no written instructions/recommendations from a medical professional regarding the recommended weight and usage were provided, nor was there any information on the use of the weighted blanket in an IEP at the facility.
Staff Person #1 has put her legs across Child #1 while sitting on the floor next to the child's on the rest mat during naptime.
The Emergency Plan did not include a contingency plan addressing continuity of operations.
Ceiling tiles and vents observed in the Older Preschool and Prekdg classrooms and bathrooms were dirty and had significant dust buildup on them.
Peeled paint/cracks and an open hole was observed on the walls in the bathroom located in the Young Preschool classroom.
The toddler playground that is fenced in separately from the other playground is not being used currently as the toddler outdoor play space was moved into the large fenced in playground, to the left of the preschool/SA play areas. Mulch is present around the preschool equipment, which is accessible to the toddlers as there is no fencing dividing the play space.
The written plan of daily activities was not posted in the School Age classroom (gym). (Corrected during inspection).
The Emergency Transportation Plan was not posted in the Young Preschool classroom (Corrected during the inspection).
The Older Playground Climber that is used by preschool and school age children has areas with rust and peeling paint on the blue pull-up bar.
“Core safety” marks citations about supervision and staff ratios, discipline and caregiver conduct, background checks, child protection and safe sleep, the topics our score weights most. Pennsylvania’s records don’t grade violations by severity. “Follow-up” marks a violation re-cited at a follow-up visit, counted once. How scores work.
Complaint investigations and reported incidents
Pennsylvania’s search lists complaint investigations in which the state found at least one violation (“verified complaints”). It does not say what a complaint alleged, or whether every violation relates to it. Incident investigations look into something that happened at the center; self-reported violations are ones the center reported to the state itself.
