In the toddler classroom the children were observed not washing hands after a diaper change. In the infant classroom a teacher was observed placing a child in the highchair for a cookie without washing the child's hands.
The caregiver in the toddler classroom stated there were 12 children in care when there were actually 10 children in care.
A caregiver in the toddler classroom was observed changing diapers with gloves without washing hands between each diaper change.
The infant classroom was observed out of ratio with eight children (4months-13 months) to one caregiver at the time of the walkthrough. This resulted in the caregiver being out of ratio by four children. NOTE: This was corrected at inspection when the caregiver returned back into the classroom.

A toddler was observed in the dining hall walking around drinking from a baby bottle. NOTE: This was corrected at inspection.
A written plan to handle transportation emergencies was not available at the time.
A seven-month-old infant was observed inside the crib drinking from a baby bottle. NOTE: Corrected at inspection when the child was taken out of the crib and held.
Two shelves in the toddler classroom were observed empty while children were waiting for diaper changes. NOTE: This was corrected when the shelves were filled with age apprpriate toys.
A trashcan in the toddler classroom was observed without a lid that contained dirty diapers that was accessible to children. NOTE: This was corrected at inspection when the trashcan was replaced.
Monthly fire drills are not being conducted and documented as required. Carbon monoxide/smoke detectors and fire extinguishers are not checked monthly and documented as required.
Gas inspection is overdue as of 7/1/22.
Two of four children's files did not have a healthcare statement included in their file.
The operation did not have a plan set up. Operation is currently in process of gathering all required information.
