Based on interviews, the licensee did not ensure that each child is accorded their personal rights. C4, C6, and C7 disclosed S1 yells at daycare children. S1 disclosed that she/he raise the voice to children. This poses an immediate risk to the personal rights of children in care. 8 Director Suk will email a declaration stating the children's personal rights have been reviewed. The declaration will include a sign in sheet confirming staff participation in the review of personal rights. The director will email this plan of correction to the LPA by the due date at the email provided.
Based on records reviewed, the licensee did not ensure that one child was visually supervised while in care. Records show the licensee admitted that a child ran into the parking lot alone while in care. 8 The director will email a copy of the sign posted in the lobby and a copy of the letter sent to the parents asking them to pick up their child inside. The director will email this plan of correction to the LPA by the due date at the email provided.
Based on interview statements, the director did not ensure documentation of the hours actually worked by facility personnel. The director stated that hours worked are not kept for S1, S3, and S4.
Based on observation and interview statements, the requirement was not met for one of ten children, which poses an immediate risk to the health, safety, and personal rights of children. C1 was left without supervision for around 5-10 minutes.

The didrecto stated the facility did not test the water for lead. The director uses a Coway system that filters tap water. This source of water has not been tested.

Three staff, two of whom were present, did not have a valid mandated reporter certificate.
