Director stated a reminder was sent to all staff regarding supervision and will email LPA a copy of the signed acknowledgement. obtained and the self reported incident report, i has been determined that C1 was left alone inside the classroom for approximately 7 minutes on 12/07/2024.. This poses an immediate risk to the health and safety of the children in care.

101223(a)(2) Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met evidence by: 1 Director stated they will provide personal rights training to all staff and provide an attendence sheet. Director will submit written POC to licensing office by the due date. Based on interviews conducted by LPA, it was determined that the facility pushed all the chairs in the cornered and were not allowed to use them, this was used as a form of punishment. This is a potential Health and Safety risk to the children in care 8
