Based on evidence gathered through observation, interview & record review, the licensee failed to ensure personal rights of person's in care, due to subject child's acts of aggression towards children and staff, which is a potential risk to the health & safety of persons in care. 8 The licensee also agrees to submit a copy of the plan implemented to address acts of aggression, including staff participation in training to support the implemented plan, and program changes to meet the needs of children in care. Proof can be submitted to the department by fax, mail or email.
Based on the interviews and record review, the Licensee did not meet the above regulation which poses an immediate Safety & Personal Rights risk to the children in care. Director understands all children must be safe and have comfortable accommodations while at the facility. Director understands if 8 in compliance with the above regulation. Written statement shall be signed & dated and is due no later than close of business on 8-27-2025. This is a repeat violation. $250 Civil Penalty assessed.
Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the file review for staff the LPA was unable to review a current Mandated Reporter certificate for S5.
Based on the observation and record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the facility tour the LPA inspected the medication stored at the facility. The LPA observed expired medication for children who are currently enrolled.
Based on interviews and records reviewed, several day care children displayed aggression towards other day care children on multiple incidents that occurred at the facility between 08/07/2024 through 12/17/2025, which is an immediate risk to the health and safety of children in care. 8 challenging behavior, setting behavior goals, and monitoring behavior progress (Part 1) and provide in-service training regarding documentation of behavioral logs, including proof of staff participation (Part 2). Licensee agrees to submit a copy of the plan of action to LPA by 02/08/2025.

Based on a tour of the Pre-K classroom and Preschool classroom, the licensee did not comply with the section cited above, LPA observed labeled bleach bottles (Bottle #3) stored behind a latched cabinet. At the time of LPA's inspection, children were napping in the Preschool classroom and both cabinets were securely latched. LPA advised Director Bottle #3 and any other toxins and/or poisons must be stored in areas with a key-lock. This poses a potential health, safety or personal rights risk to persons in care.
Based on inspection of the outdoor play area , the licensee did not comply with the section cited above, based on LPA Hogue observation of the preschool playground had sand and wood chips on the playground equipment, including playground stairs and playground steps.
