Based on record review, the licensee did not comply with the section cited above in that 2 out of 5 children's records (Child 4 [C4] and Child 5 [C5]) did not have a signed consent form for emergency medical treatment. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview, observation, and record review the licensee did not comply with the section cited above in that the licensee had the space provided for child care center children (preschool age children) in use by the school-age child care center children. School-age children and preschool age children were not present in the same rooms during this visit. This poses/posed a potential health, safety, or personal rights risk to children in care.

Based on the observation and interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the inspection of the pool area the LPA was unable to observe a body hook. The Director stated a body hook was not available for review.
